Vaginismus is defined as painful spasms of the vaginal muscles due to local hypersensitivity during attempted coitus.
When an attempt is made to examine the patient per vaginam, there is involuntary spasm of the perineal and thigh adductor muscles.
Etiology (Causes)
- No organic lesion found.
- Psychosexual Anxiety
- Sexual Phobias – Fear of sexual intercourse
- Primary & Secondary Vaginismus
- Vulvitis
- Laceration of Hymen
- Tender Scars on Perineum
- Narrow Vaginal Introitus
Clinical Features
- Severe pain during intercourse (Dyspareunia)
- Refusal or fear of sexual activity
- Involuntary contraction of vaginal muscles
- Emotional distress related to intercourse
1. Psychological Therapy
- Counseling to build confidence in the patient
- Sex education for both partners
- Desensitization therapy
2. Physical Therapy
- Gentle vaginal dilation under anesthesia
- Daily introduction of vaginal dilators for 1-2 weeks
3. Surgical Intervention
- Surgical hymenotomy (if
hymen is tight)
Keynote Symptoms of Vaginismus1. Involuntary spasms of vaginal muscles preventing penetration
2. Pain or burning sensation during attempted intercourse
3. Fear, anxiety, or past trauma associated with sexual activity
4. Tightening of perineal and thigh muscles during examination
5. Possible history of painful menstruation or recurrent vaginal infections
Indications:
- Painful intercourse due to dryness and vaginal spasms
- Aversion to sex, with irritability and mood swings
Keynote Symptoms:
- Indifference to sexual activity.
- Feeling of a weight or dragging sensation in the pelvis.
- Vaginal dryness leading to painful intercourse.
- Better with vigorous exercise.
- Worse from cold air and during pregnancy.
Indications:
- Vaginismus due to emotional trauma, grief, or anxiety
- Fear and hypersensitivity associated with sexual activity
Keynote Symptoms:
- Hysterical behavior with mood swings.
- Feeling of a lump in the throat when anxious.
- Spasmodic contraction of muscles, including the vagina.
- Worsens with emotional stress and suppressed emotions.
- Better from deep breathing and distraction.
Indications:
- Hyper-sensitivity of the vagina, causing painful intercourse
- Spasmodic contractions of the pelvic muscles
Keynote Symptoms:
- Increased sensitivity of genital organs.
- Feeling of superiority and pride.
- Alternating sexual desire with aversion.
- Worse in the evening and from emotional tension.
- Better with slow movement and relaxation.
Indications:
- Vaginismus due to suppressed sexual trauma or abuse
- Deep-seated fear or guilt related to intercourse
Keynote Symptoms:
- Suppressed anger and emotional sensitivity.
- Fear of being touched or approached.
- Burning sensation in the urethra after urination.
- Worse from suppressed emotions and humiliation.
- Better from warmth and rest.
Indications:
- Severe pelvic muscle spasms, leading to difficulty in penetration
- Vaginismus due to nerve hypersensitivity
Keynote Symptoms:
- Involuntary muscle contractions and nervous tension.
- Difficulty relaxing pelvic muscles.
- Painful intercourse with burning sensation.
- Worse in cold, dry weather and from emotional distress.
- Better from warm applications and gentle pressure.
